A 52-year-old female misdiagnosed as “disc herniation” at the first visit. After 4-month conservative treatment, the clinical symptoms aggravated. a, b, c T1,T2-weighted and STIR MRI showed inflammation in L5–S1 vertebral bodies and intervertebral disc. Spinal epidural abscess and inflammatory granuloma extend to anterior epidural space, resulting in spinal stenosis at L5-S1 level; (d) Transverse T2-weighted MRI showed left nerve root and dural sac compression; (e) postoperative X-ray showed intervertebral bone grafting and instrumentation; (f, g) 6-month and 36-month postoperative X-ray showed solid bony fusion. (h) the last follow-up CT showed solid intervertebral and facet joint bony fusion
